Player Registration Form

If you do not wish to register online, please print and complete the following Player Registration Form and bring to one of the available in-person registrations. If you are unable to print, forms will be available at the on-site registration. Thank you.

Volunteer Registration Form

We are currently not accepting online Volunteer form applications. If you wish to volunteer, please print the 2023 New Volunteer Form and bring to one of the in-person registration tables or email to [email protected]. For all *new* JLL volunteers (ie. did not volunteer during the 2012 Spring season), social security number is required to conduct background check. Any *new* volunteers that do not enter social security, the forms will not be accepted.

If you are a returning volunteer, please complete the following 2023 Returning "Basic" Volunteer Registration Form. At this time, the social security number is not required for returning volunteers.

Please provide a photocopy of your driver's license. All volunteer forms must include a valid driver's license, or the form will not be accepted. Thank you.
